As intensive care unit specialists in New York City, we are concerned about the increasing rates of COVID-19 infection, which may once again overwhelm our hospitals, in parts of the United States.

The severity, morbidity and mortality of COVID-19 must be re-emphasized to all, both young and old, as it spares neither. To avoid a catastrophic repeat of the initial surge, we recommend a population-wide intervention ā€” a significant increase in the use of N95 masks ā€” that might allow for a safer reopening of the U.S economy.

SARS-CoV-2 was thought to be primarily transmissible via large fluid-filled droplets generated by coughing or sneezing. Those droplets usually travel short distances before falling and will not reach another person practicing social distancing nor penetrate standard (cloth or surgical) masks.

However, it has recently been determined that a major mode of transmission of SARS-CoV-2 is via aerosol droplets, exhaled by presymptomatic, asymptomatic or symptomatic persons. These small aerosol particles remain airborne indoors for extended periods and can infect those nearby who inhale them into their lungs.

We believe that the lack of emphasis placed on the prevention of aerosol droplet transmission is a major contributor to the rising case numbers in many areas of our country.

What can be done to prevent the inhalation of these aerosols and therefore reduce the spread of SARS-CoV-2? The only mask that can prevent aerosol-size droplet inhalation is an N95.

Too few Americans wearing masks

Here is the key: Standard masks, although only partially effective in blocking inhalation of aerosols compared with the near perfect blocking performance of medical grade N95 masks used in hospitals, are highly effective at trapping the large droplets exhaled by infected people.

These large particles downsize to aerosol size after emission when they undergo evaporative loss. Although standard masks are imperfect in both blocking or trapping, their combined performance when worn by both infected and noninfected persons leads to a low likelihood of transmission.

The key point is that, for standard masks to be effective, there needs to be near universal wearing of these masks by all persons when in any poorly ventilated, air-recirculated, confined indoor, or highly congested outdoor environment. Conversely, emphasizing mask wearing in fresh air outdoor settings has no epidemiologic support and thus makes little sense.

Unfortunately, in some parts of the United States, the proportion of citizens routinely wearing standard masks in at-risk environments is nowhere near what's required to prevent spread. Would it perhaps be better for everyone to wear masks proudly in all public spaces (with air gaps, breathing holes, mesh for comfort and health) with a short url printed on them to a very simple and friendly website showing all of the contagion study results/ virology control experiments etc?

The problem with non-mask wearers was always that they could never communicate their reasons for not wearing a mask. But what if masks became the new 'subversive' T-shirts......free billboards for the masses?

Unfortunately, we live in a country full of sheep Peggy. After all you have done to help humanity during this despicable and satanic hoax, those of us who understand are still in the minority. I live in a small city called Massillon, Ohio. I am surrounded by neighbors who fell for the hoax hook line and sinker 5 years ago despite any and all information I would present to them. All they watched (and still do) is the MSM propaganda machine. They all took shots (and now have health problems) and they ignored every video or article that I showed them from so many people who know the facts, including you. The election will be stolen again in no uncertain terms and again, the sheep will sit back and do nothing. Apathy and ignorance is what put us in this position and sadly, I won't say here what it will take to save this country, but it certainly is not voting or complying with illegal edicts. And unless Americans understand what the true patriots understood about tyranny over 240 years ago, the American Experiment will come to an end.

Let's get down to the basics. Ordering medical treatment, medical device, medication, or therapy, is the purview of a medical doctor or physician. The unelected bureaucrats ordering such medical intervention is essentially practicing medicine without a license. That is a crime! FDA, CDC, NIH, FAUCI et al, Governors, State Health Departments, they all need to be held to account for their criminal actions. Government at any level has no business in the practice of medical care. Indict and Defund them all!

My son was severely punished by his ā€œCatholicā€ high school in a Feb 22 mask protest. His video of kids cheering went viral and was reposted by conservatives. The school publicly shamed him and after 6 weeks when the isolation and shame was too much we had to withdraw him from the school. My one regret is going along with the hogwash for 300 days but it was the only way to try and keep our lives as normal as possible and it was anything but normal. I lost a lot of ā€œfriendsā€ and coaches, teammates and 95% of the people we associated with never spoke to us again. The blind disregard for facts and truth and the adherence to a political agenda was appalling. I pray daily that the evil doers at St. Viator HS in Arlington Heights, Illinois and the community members who allowed public shaming of students are held accountable by God. I never want to see this happen again. As a parent I still deal with the trauma daily and although my son is a functioning college student today, itā€™s been a long, painful road.
